Long-term Care
At Lakeview Gardens, we are implementing our philosophy in person-centered care by building two brand new small home cottages for long-term care and one for our assisted living facility. Each home will consist of 12 private bedrooms, each equipped with its own European-style bathroom (shower included). All residents are encouraged to decorate their rooms to their personal preference, but must get approval from administration. Adding this element of individualization is the wave of the future for long-term care facilities and our elders. Lakeview Gardens is proud to be one of the few facilities in Oregon adopting this concept.
Meals
In our small home community, meals are prepared in the home, which allows our residents an opportunity to assist with cooking meals, baking snacks, and setting the table just like they would in their own homes. Meal hours will be flexible and allow residents to eat when they would like.
Laundry
Each home will be equipped with high-end washers and driers that will allow our staff do all residents’ personal laundry within the home.
Visitors
Within the long-term care facility, we will have 24-hour visitation hours. Since this is our residents’ home, visitors will be required to ring the doorbell. Our attentive staff will allow entry to visitors upon our residents’ request. We will have a sleeper sofa located in each home’s den for out-of-town visitors for brief stays or for hospice residents’ families.
Individualized care planning
At Lakeview Gardens, we understand that each person is different in many ways. This is why we have implemented individualized care planning. Stemming off of our person-centered care philosophy, individual care planning means upon admission, our staff ask specific questions about each resident’s life to create the best routine for every new resident. This can be changed as often as needed in order to create the best quality of life for each person at Lakeview Gardens.
Activities
We pride ourselves in offering an abundance of activities that allow our residents to keep active and to maintain or improve their quality of life. We provide activities such as bingo, gardening, visiting pets, arts and crafts, holiday festivities, and much more.

Lakeview Gardens Assisted Living’s approach to Assisted Living is founded on the philosophy of the small home concept. You can depend on our compassionate team for excellent care and individualized services.
Dining
When you move into Lakeview Gardens Assisted Living, you have the freedom to choose your dining experience. Whether you want to dine in your room or eat with your friends in the dining room, you will enjoy our fresh, delicious home cooked meals. We offer multiple options for each meal and are flexible with your schedule.
Activities
Lakeview Gardens Assisted Living is designed to make it easy for you to participate in a variety of activities that are individualized to suit each person. Our activities are designed to engage your body and mind with emphasis on group and individual activities. Whether it’s a movie in the living room, spending time in our garden or group exercises, you are sure to find activities that interest you.
How do I know if Assisted Living is right for me?
If you are requiring assistance with activities of daily living such as management of your medication, personal hygiene, dressing or grooming, then assisted living is probably a good fit. All residents will meet with qualified health services personnel for a thorough assessment prior to move in.

Rehabilitation Services, Activities and Social Services
Our superlative physical rehabilitation team provides excellent care that is driven by each resident’s goals. The team works diligently to ensure residents are able to reach their highest level of independence. This often includes providing continuous education to residents and their families to ensure a safe return home. Our skilled physical rehab interdisciplinary team takes care to develop a specialized service plan for every resident to ensure each is able to achieve his or her individual goals.
The physical rehab team includes skilled professionals in the following disciplines:
Physical therapy (PT): Physical therapists create individual treatment plans to match each person’s goals, helping people improve and maintain function and quality of life by partnering in their own care. A physical therapist diagnoses and treats existing health conditions, but also provides residents and families with customized plans to help prevent health problems before they occur.
Occupational therapy (OT): Our occupational therapy team works closely with residents and families to teach or reteach skills that are necessary for activities of daily living. We have an excellent OT team that takes pride in their work and in getting people back with their loved ones at home.
Speech therapy (ST): Speech therapy is crucial to help residents regain their oral motor skills. Therapists work closely with stroke victims and dementia residents to enhance their swallowing skills and ability to chew their food, to regain muscular strength in and around their mouths, and much more. Speech-language pathologists not only work toward improving swallowing performance and increased awareness of what structures are involved in swallowing, but also determine the safest diet consistency and any compensatory strategies a resident may require. Speech therapy may also include cognitive, voice, fluency, or language assessment and treatment.
Restorative nursing: Our rehab team works in congruence with nursing staff by developing care plans and providing training and education to continue the journey toward each person’s goals. With instructions and supervision by our rehabilitation team, our staff provides a range of interventions and activities to keep residents at an optimal level of physical mobility and independence
