Our Services

Lake Health Clinic, a patient-centered primary care home, offers a comprehensive array of health and wellness services to meet the needs of every member of your family, from infants to seniors. Whether it be an annual physical, immunizations for travel abroad, or a strep test, you will find what you need here.

We are strongly committed to our community, and work diligently toward integrating traditional and cutting-edge medical practices to provide the best care possible. Our doctors, physician assistants and nurse practitioners work collaboratively, supported by a team of skilled medical assistants. Most patients develop a primary relationship with one provider in addition to long-standing relationships with others through use of our convenient walk-in clinic.

Our capable and friendly staff will schedule your appointments, obtain any necessary referrals and prior authorizations, and manage the insurance billing and follow up. They will help you to navigate the sometimes-difficult world of healthcare in an efficient and timely manner.

We accept all insurance plans in the area for your convenience and are accepting new patients. Clinic hours are 7:00 a.m. to 5:00 p.m. Monday through Friday.

Please note new policy regarding copayment, past due balance and no-show fee:

Insurance copay is due at each visit.

Self-pay patients are required to make a minimum $75 payment at each visit unless other arrangements are made with Patient Services prior to visit.  Self-pay patients are further responsible for the remainder of the visit cost above the $75 payment.  

Past due balances must be paid in full or payment arrangements made with Patient Services prior to the next visit.

Lake Health Clinic charges a no-show fee of $25 for missed scheduled appointments (not applicable for Medicaid patients).  Payment of the late fee is required to be paid in full prior to the patient’s next scheduled visit.

Family Medicine

Your primary care provider will ensure that you are getting all the appropriate preventive care and diagnostic tests based on your age and situation. Nutrition, exercise, and lifestyle choices can have a major impact on your health. When your care requires a referral to a specialist, you will know that your care will be coordinated effectively.


Whether you are looking for your newborn’s first pediatrician or you are new to the area and hoping for a great pediatric practice for your child, you will be welcome. Our kid-friendly waiting room, exam rooms, and a warm, friendly atmosphere will greet you and your family. Our doctors will ensure your child’s development is on track with regular physicals, immunizations, flu shots and of course be your partner when your child picks up a sniffle at school.

Walk-in Clinic

When you are suffering from an unexpected illness or injury, or want general medical attention without an appointment, you are invited to utilize our convenient walk-in clinic. Our providers are equipped to handle a wide range of urgent medical conditions and injuries, routine illnesses, and general medical services.

We offer a Walk-In clinic for same-day appointments Monday to Friday, from 7:00 am to 4:00 pm, with a break for lunch at 12:00 pm. Walk-ins resume promptly at 1:00 pm. Walk-in clinic is first come first serve, and times and dates of service are subject to change.

CDL physicals are done in the Walk-in Clinic on Fridays from 7 a.m. to 12 p.m. only.

Care provided at Lake Health Clinic

We have six providers offering the full range of healthcare services to our community. Our services include all urgent care, routine family practice, and intermediate care without the long waits that accompany the emergency room and at a cost savings to you. We accept all insurance plans in the area for your convenience and many of our providers are accepting new patients.


In 2022, Lake Health Clinic instituted a $25 “no-show” fee due to the large number of appointment no-shows. This fee is assessed to all patients and is not payable with insurance. Our policy is available upon request.

Lake Health Clinic provides healthcare to a large population of patients, and every visit that isn’t cancelled at least 24 hours in advance could have been an opportunity for another patient to be cared for.

What Is Medicated-Assisted Treatment?

Medicated-Assisted Treatment (MAT) is the use of FDA- approved medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders.

We also offer an extensive specialty practice to our patients so that they can be seen locally, rather than traveling out of town.

Financial Assistance

Lake Health Clinic (LHC) is committed to providing financial assistance to people who have healthcare needs and are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay, for medically necessary care based on their individual financial situation. LHC provides financial assistance for people with financial need by waiving all or part of the charges for services provided by LHC. LHC will provide, without discrimination, care for emergency medical conditions to individuals regardless of their eligibility for financial assistance or for government assistance.

Household Income: Household income is determined using the Census Bureau definition, which uses the following income Determined on a before-tax-basis, when computing federal poverty guidelines:


  • Earnings (Wages, Bonuses, Tips, part or full time)
  • unemployment compensation
  • workers’ compensation
  • Social Security (Excluded if sole source of income)
  • Supplemental Security Income
  • Public assistance,
  • Veterans Retirement payments
    • Veterans Disability Benefits are not included as proofs of income.
  • Survivor benefits,
  • Pension or retirement income,
  • Interest, dividends, rents, royalties,
    • Capital gains or losses are excluded
  • Income for estates and/or trusts,
  • Educational assistance (excludes tuition and materials cost),
  • Alimony,
  • Child support (if the child for which support is received is the patient)
  • Assistance from outside the household, and other miscellaneous sources;
  • Non-cash benefits (such as food stamps and housing subsidies) are excluded;

If a person lives with a family, includes the income of all family members (Non-relatives, such as housemates, are not included).
