Lake Health Clinic, a patient-centered primary care home, offers a comprehensive array of health and wellness services to meet the needs of every member of your family, from infants to seniors. Whether it be an annual physical, immunizations for travel abroad, or a strep test, you will find what you need here.

We are strongly committed to our community, and work diligently toward integrating traditional and cutting-edge medical practices to provide the best care possible.ย Our doctors, physician assistants and nurse practitioners work collaboratively, supported by a team of skilled medical assistants. Most patients develop a primary relationship with one provider in addition to long-standing relationships with others through use of our convenient walk-in clinic.

Our capable and friendly staff will schedule your appointments, obtain any necessary referrals and prior authorizations, and manage the insurance billing and follow up. They will help you to navigate the sometimes-difficult world of healthcare in an efficient and timely manner.

We accept all insurance plans in the area for your convenience and are accepting new patients. Clinic hours are 7:00 a.m. to 5:00 p.m. Monday through Friday.

The medical home philosophy focuses on primary care that is patient-centered, all-inclusive, team-based, coordinated, easily accessible, and dedicated to quality and safety.

The medical home philosophy is a widely accepted model for how primary care should be organized and delivered throughout the health care system. The philosophy encourages providers and care teams to meet patients where they areโ€”whether their health care needs are simple or complex, and embraces the idea that a medical home is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patientโ€™s needs.

Family Medicine

Your primary care provider will ensure that you are getting all the appropriate preventive care and diagnostic tests based on your age and situation. Nutrition, exercise, and lifestyle choices can have a major impact on your health. When your care requires a referral to a specialist, you will know that your care will be coordinated effectively.

Pediatrics

Whether you are looking for your newbornโ€™s first pediatrician or you are new to the area and hoping for a great pediatric practice for your child, you will be welcome. Our kid-friendly waiting room, exam rooms, and a warm, friendly atmosphere will greet you and your family. Our doctors will ensure your childโ€™s development is on track with regular physicals, immunizations, flu shots and of course be your partner when your child picks up a sniffle at school.

Walk-in Clinic

When you are suffering from an unexpected illness or injury, or want general medical attention without an appointment, you are invited to utilize our convenient walk-in clinic. Our providers are equipped to handle a wide range of urgent medical conditions and injuries, routine illnesses, and general medical services.

We offer a Walk-In clinic for same-day appointmentsย Monday to Friday, from 7:00 am to 4:00 pm, with a break for lunch at 12:00 pm. Walk-ins resume promptly at 1:00 pm. Walk-in clinic is first come first serve, and times and dates of service are subject to change.

CDL physicals are done in the Walk-in Clinic on Fridays from 7 a.m. to 12 p.m. only.

Care provided at Lake Health Clinic

We have six providers offering the full range of healthcare services to our community. Our services include all urgent care, routine family practice, and intermediate care without the long waits that accompany the emergency room and at a cost savings to you. We accept all insurance plans in the area for your convenience and many of our providers are accepting new patients.

In 2022, Lake Health Clinic instituted a $25 "no-show" fee due to the large number of appointment no-shows. This fee is assessed to all patients and is not payable with insurance. Our policy is available upon request.

Lake Health Clinic provides healthcare to a large population of patients, and every visit that isn't cancelled at least 24 hours in advance could have been an opportunity for another patient to be cared for.

What Is Medicated-Assisted Treatment?

Medicated-Assisted Treatment (MAT) is the use of FDA- approved medications, in combination with counseling and behavioral therapies, to provide a โ€œwhole-patientโ€ approach to the treatment of substance use disorders.

We also offer an extensive specialty practice to our patients so that they can be seen locally, rather than traveling out of town.
