Lake District Hospital and Lake Health Clinic are requiring copay at the time of patient visit
Lake District Hospital copays can be paid at the main lobby reception desk. For information regarding required amounts, please call 541.947.2114.
Lake Health Clinic has requirements as follows:
Insurance copay is due at each visit
Self-pay patients are required to make a minimum $75 payment at each visit unless other arrangements are made with Patient Services prior to visit
Past due balances must be paid in full or payment arrangements made with Patient Services prior to the next visit
The Clinic's no-show fee of $25 is required to be paid in full prior to the patient's next scheduled visit.
Lake District Hospital and Lake Health Clinic require patients to present insurance and identification cards at every visit

Notice to the Public - Face Masks Are Now Optional
Oregon Occupational Health and Safety Administration (OSHA) has suspended its permanent Rule: 437-001-0744 Rule Addressing COVID-19 Workplace Risks, dated 09/12/2022, requiring masking in healthcare facilities.
Likewise, the Oregon Health Authority (OHA) has lifted the masking requirement for all healthcare facilities as of April 3, 2023. Patients and visitors to Lake Health District, Lake District Hospital, and all its affiliated elements are no longer required to wear masks within the facilities.
While face masks are no longer required, please respect those who choose to wear them.

Lake Health Clinic institutes "no-show" fee
In 2022, Lake Health Clinic instituted a $25 "no-show" fee due to the large number of appointment no-shows. This fee is assessed to all patients and is not payable with insurance. Our policy is available upon request.
Lake Health Clinic provides healthcare to a large population of patients, and every visit that isn't cancelled at least 24 hours in advance could have been an opportunity for another patient to be cared for.
