Lake District Hospital (LDH) is committed to providing financial assistance to people who have healthcare needs and are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay, for medically necessary care based on their individual financial situation. LDH provides financial assistance for people with financial need by waiving all or part of the charges for services provided by LDH. LDH will provide, without discrimination, care for emergency medical conditions to individuals regardless of their eligibility for financial assistance or for government assistance.
Household Income: Household income is determined using the Census Bureau definition, which uses the following income Determined on a before-tax-basis, when computing federal poverty guidelines:
Includes:
- Earnings (Wages, Bonuses, Tips, part or full time)
- unemployment compensation
- workers’ compensation
- Social Security (Excluded if sole source of income)
- Supplemental Security Income
- Public assistance,
- Veterans Retirement payments
- Veterans Disability Benefits are not included as proofs of income.
- Survivor benefits,
- Pension or retirement income,
- Interest, dividends, rents, royalties,
- Capital gains or losses are excluded
- Income for estates and/or trusts,
- Educational assistance (excludes tuition and materials cost),
- Alimony,
- Child support (if the child for which support is received is the patient)
- Assistance from outside the household, and other miscellaneous sources;
- Non-cash benefits (such as food stamps and housing subsidies) are excluded;
If a person lives with a family, includes the income of all family members (Non-relatives, such as housemates, are not included).
